Questionnarie?

Dear Classmates,

While it may seem like just a moment ago, it has been quite a number of years since we took that fateful walk to the podium to receive our diplomas. It’s been a while since we last met the very people we could not live without.

Since graduation, we have all done a lot of living and we would like to get some information from you so we can share your whereabouts with everyone. We hope you feel comfortable sharing the life you've led as we are eager to see how things turned out for everyone.

The answers you provide will be used to create a memory book/CD for our class. The questions are very detailed and we hope you feel comfortable enough to provide detailed answers. The more information we have, the better a memory book we can create for everyone.

The memory book/CD will be a collaborative effort that promises to be fun and engaging. Our hope in creating it is that you will want to look through it for years to come (Or at least till the next class reunion).

It is time to get to work!!  Okay, it’s probably been a while since you’ve been given homework, but this assignment is important and fun!!  You won’t be graded on it, but you still need to finish it.  By all mean, don’t put it off till the last minute.

Cut & Paste this into a Word Document or your email.  Email questionnaire to sgoff@bristolharbour.com, Subject: 20th Class Reunion Questionnaire or mail it in with your deposit.

If for some reason you cannot attend the reunion (and we REALLY would like everyone to attend) we still want all of your information!  Fill out the questionnaire just like everyone else.

As soon as we start receiving your completed questionnaires, we will begin the process of putting together your Memory Book/CD.

Photos?  We would love to have a photo (or two or three) from everyone.  We are looking for a current picture, family picture and finally, we would like you to look through old photos.  Do you have any old photos from our days at NCS that you could share?  If you do, please send them to us (or if you are in the “digital age”, email them to us.)  Don’t forget to write a caption on the back, include the approximate date of the photo and your name (so we know who they belong to). We will return them at the reunion.  Send as many as you wish and for those who are out of town and can’t make it, if you would like to send a video clip that would be great!!  All video clips welcomed, even if you are coming!!

If you have any questions, please feel free to reach out to me here or email me at Sgoff@bristolharbour.com.

Now, open up your favorite beverage, enjoy some of the 90's music at the bottom of the BLOG (that should bring back some memories) and get ready to type away!!!

The information you supply below will used to make a Memory Book/Cd.
___I grant permission to print answers from questions on this form in the memory book.
___I grant permission to share my contact information with other classmates.
